Maple Family Health Team is a group of 27 physicians who work alongside Interdisciplinary Healthcare Providers to deliver primary care to patients in the Kingston area.
We have an opening for an experienced Community Service Worker to join our multi-disciplinary team on a temporary basis. While this position is temporary, there is a possibility for extension.
Position Description:
The Maple FHT community services worker works with complex patients, advocating for and linking them to community resources that will resolve unmet social health needs such as housing, income security, food security, transportation, literacy and social isolation.
Qualifications:
- College diploma in Social Services,
- 1-2 years of practical experience,
- Awareness and understanding of the community supports available in Kingston and area,
- A strong understanding of social determinants of health,
- Understanding of relevant legislation (e.g. Health Care Consent Act, Substitute Decisions Act, Personal Health Information Protection Act, Child and Family Services Act, etc.), and
- Demonstrated commitment to continuous learning.
Representative duties and responsibilities:
- Apply a patient-centered approach to work with complex patients in determining the social health action plan,
- Apply a therapeutic relationship framework, non-judgmental attitude, warmth, respect and caring with patients and their families,
- Facilitate social health community-based referrals,
- Facilitate application processes with government and community services, and
- Liaise with community supports to advocate for patients and to gather information about their services which you will share with Maple healthcare providers.
Required skills:
- Communicate with other members of the health care team to plan, implement, coordinate and evaluate,
- Proven ability to provide leadership in the social health domain to optimize patient experiences,
- Proven ability to build and maintain relationships across community sectors,
- Proven skills in organization, multi-talking, problem solving and collaboration,
- Knowledge of community resources available to patients in the City of Kingston,
- Strong oral and written communication skills and interpersonal skills,
- Proficiency in computer software programs (e.g. Excel, Word, Electronic Medical Records),
- Demonstrated commitment to quality improvement,
- Current and satisfactory Criminal and Vulnerable Persons Check, and
- Access to reliable vehicle with valid driver’s license and insurance.
Information about the position:
- Full-time (5 days per week) or part-time (please indicate your preference in your application)
- Daytime shifts (no evenings or weekends)
- Term position, ending March 31, 2025. There is a possibility of extension.
- Optional HOOPP enrolment
- The incumbent must be compliant with Maple’s COVID vaccine policy which requires full vaccination.
